Who has access
Clinic staff, as part of their work. Our appointment and communications platform provider, which hosts the form and messages on our behalf. Its servers may be located outside Quebec. We do not share your information with anyone else, unless the law requires it.
Retention
Form information is kept as long as needed to answer you and manage your appointments. If you become a patient, it becomes part of your record, kept according to the rules of the Ordre des dentistes du Québec.
